- What You Need: Proof of Florida seasonal or permanent residence; medical records and proof of condition; signed release of information form; certification from a Florida-licensed medical marijuana program physician such as Triverde Health’s Dr. Ycaza.
- You must see the doctor within 30 days of the end of the 7th month. The card is valid for 12 months.
- Seasonal Florida residents who live here more than 31 days are eligible.
- You can purchase medication once you get your approval email in ten days. Your card comes in the mail within 2-3 weeks.
